Skin aging: The basic facts
Aging is unavoidable fact of life – everyday we are all getting older. However, it is not age itself that bothers us, but the visible signs of aging that can make us feel old despite the fact that we feel young and vital. The health and look of our skin – whether on our face or on our body - plays a significant part in how old we feel and is seen as the most common indicator to others of our age. The skin is the body’s largest organ, our skin is hugely influenced by our body’s general well-being – if our body is not healthy, it is often reflected by our skin.
So what are the signs of aging skin?
Thinning and wrinkling of the skin: When we age, thinning of the skin occurs as the rate of cell production slows in the epidermis. The dermis may also become thinner, and these changes in both layers result in skin being more likely to crepe and wrinkle. Older skin is often described as being more thin paper-like
Sagging: Sagging occurs in ageing skin as less elastin and collagen are produced. With this lower level of elastin and collagen, skin eventually yields to the forces of gravity, causing sagging and drooping
Age spots: Pigment cells contained in the epidermis called melanocytes, tend to increase in certain areas, mainly those areas like the backs of hands which have been exposed to the sun, and cluster together, forming what are known as age or liver spots
Dryness: aging skin has fewer sweat glands and oil glands. This can make skin more prone to dryness-related conditions such as roughness and itching
Infra Red Laser Skin Tightening
In the last 10 years various non-invasive and non surgical treatments modalities have been developed for aging skin. Most of the advanced dermatology clinics in the US and UK are using lasers, radiofrequency (RF), IR lasers and a combination of the above to treat aging skin. All these methods are aiming to heat the deep skin layer in order to shrink redundant or lax connective tissues through the mechanism of collagen denaturation.
Collagen molecules are produced by skin cells called fibroblasts which synthesize three polypeptide chains that wrap around one another in a triple helix. The discovery of thermal shrinkage of collagen fibers begins with a denaturization of the triple helix of the collagen molecule. When collagen is heated, the heat-labile intra-molecular cross-links are broken and the protein undergoes a transition from a highly organized crystalline structure to a random, gel-like chemical state. Collagen thermal shrinkage occurs through the cumulative effect of the ‘unwinding’ of the triple helix, due to the destruction of the heat-labile intra-molecular cross-links, and the residual tension of the heat-stable intermolecular cross-links. Heated fibroblasts cells are also implicated in neo collagen formation and subsequent tissue remodeling, which can also contribute to the final cosmetic result. The precise heat-induced behavior of connective tissues and the extent of tissue shrinkage are dependent on several factors which include the maximum temperature reached in the skin tissue, exposure time, tissue hydration and tissue age.
How does the Titan Infra-red laser work on aging skin?
Titan™ utilizes infrared light to heat the dermis well below the skin’s surface by heating up the water content in the skin . This heating can cause immediate collagen contraction by the denaturization of collagen. During the procedure, the outer skin’s surface (epidermis) must be protected through continuous cooling by means of the hand piece. After the procedure, new collagen growth results in further tightening and the development of a more youthful, complexion with less fine lines, folds or looseness of the skin. The Titan treatments tend to benefit a wide range of patients and can be used with sensitive skin conditions. Patients with dark skin should avoid the treatment.
Who can be treated?
Since the Titan™ procedure is heating tissue under the skin’s surface most of the patients can be treated. Men and women of all ages have been successfully treated.
Who should not undergo this procedure?
Women who are pregnant or breastfeeding are not suitable candidates. Since the Titan™ procedure is heating tissue under the skin’s surface by Infra-red light, dark skin absorb most of the energy in the uppermost skin layer and can cause burns.
Large areas of loose or redundant skin, such as after massive weight-loss may be better suited to surgical approaches.
Also, until further studies have been carried out it is advisable not to have treatment directly over dermal filler products.
How many treatments will I need?
The length of one treatment session depends on the size of the area(s) being treated, but can take between ½ and 1 hour. Expect 1 hour or longer for treating an area such as the tummy or full face and neck.
Two treatments four weeks apart are recommended for getting anti aging effect. You will be advised by the practitioner as to the number and frequency of treatments recommended for optimal results.
How long do the results last?
The desired results last approximately up to 12 months. A maintenance treatment is required at least once a year.
What happens after the treatment? Are there any side effects?
There are very few documented side effects to date, although mild swelling and redness post-treatment is to be expected. This may last 24 hours but will normally resolve after 1 - 2 hours.
In very rare cases, small blisters, or burns may occur on the skin, which should heal in under a week. This is generally considered to be a risk in more boney areas of the body, such as on the forehead or along the jawbone.

Prices with the Titan™ device for example can start from approximately £700 per one session for areas such as the whole face or abdomen (tummy). Larger areas such as the face and neck combined start from approximately £1000. Smaller areas such as the neck or part of the face (upper or lower) range from £500 - £500 per session. Bear in mind you need at least 2 treatment sessions and maintenance treatment once every year.
